VillageHealth is a dedicated team of professionals focused on integrated care management for patients with CKD and ESRD. Our focus is on the total patient. Our VillageHealth Nurses (VHNs) are the focal point of integrated care. Specially trained to care for people with kidney disease, diabetes, heart disease, high blood pressure, and other related conditions, VHNs serve as both coordinators and coaches on key renal care issues. VHNs partner with the patient, nephrologist and health care team to help manage patient outcomes, medications, nutritional needs and health progress.
