VillageHealth is a dedicated team of professionals focused on total care management for patients with CKD and ESRD. Our focus is on the total patient. Our Mission is to improve our patient’s quality of life and benefit society through integrated care. Our VillageHealth Care Management Nurses are specially trained to care for people with kidney disease, diabetes, heart disease, high blood pressure, and other related health conditions. They partner with the patient, nephrologist and health care team to help manage patient outcomes, medications, nutritional needs and health progress.
