Position Description

This position drives division-level growth, ops, clinical and promotional efforts for Home dialysis and partners with the SunRays First Team to co-manage the divisional budget and forecast for Home operations.  Position ensures and may provide ongoing training and education within the assigned area as necessary and provides quality outcomes support as needed.
Position has shared supervisory responsibility in collaboration with Regional Directors for recruitment, hiring, retention, performance development, performance management, salary adjustments and termination of Home-facing teammates within the division.
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive.
  • Drive clinical and operational outcomes
      • Ensure that compliance and audit requirements are met
      • Achieve productivity ratios as well as budget targets for operational goals through the use and/or implementation of the appropriate tools
      • Develop improvement plans for outliers
  • Support market development and growth
    • Develop strategic growth initiatives as needed
    • Support the integration process of acquisitions with the region as needed
  • Develop and maintain a collaborative relationship between the assigned facility FAs, ROD and Group Director, Home Therapies to ensure that clinical and financial goals are monitored and met
  • Other duties and responsibilities as assigned by DVP and/or Group Director, Home Therapies
  • Travel Required: up to 70%, mainly within the division by car
Daily responsibilities and expectations for the position.
Financial Management
  • Participate in budget formulation; review staffing, scheduling needs and patient loads
    • Assist designated ROD/FA with budget management
    • Supervise inventory management including purchasing, product selection and supply usage
  • Ensure patient and treatment charge information and other related data are accurate and submitted to the DaVita Corporate Office in timely manner
Oversee Patient Care Management
  • Recommend, implement, and administer policies, procedures and resources to ensure safe, appropriate treatment of patients
  • Oversee nursing and technical services for patient treatment; ensure that appropriate adjustments in treatment plans are developed, implemented and comply with state and federal governing agencies
  • Ensure development and implementation of policies, procedures and resources to help patients assume responsibility for their own health maintenance
    • Develop rapport with patients and manage patient concerns appropriately
  • Ensure communication and continuity of planned care is maintained for patients transferred to other DaVita facilities
  • Maintain open communication with all healthcare providers concerning patient care
Risk Management & Quality Assurance Compliance
  • Assist with the preparation of data summaries for internal audit purposes
  • Ensure compliance with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (including but not limited to HIPAA, CMS, OSHA or other governing agencies)
  • Ensure compliance with state or federal regulations of the Teammate Injury Prevention and Safety Training program and assist as necessary
  • Assist with the creation and implementation of any corrective actions needed as a result of internal or external audits or as required by CMS, OSHA, or other governing agencies
  • Participate in quality assurance programs and/or CQI projects as needed
Facility & Equipment Management
  • Ensure appropriate and adequate equipment and supplies are proactively maintained 
  • Assist with identification and plans for prompt corrective actions required for any issues with building safety or equipment and systems
  • Participate in the development and periodic revision of policies and procedures with the Regional Operations Director and/or Medical Director to ensure consistent, efficient, and safe operation of the facility
Teammate Management
  • Plan, organize and supervise nursing, technical and biomedical teammates to ensure high quality patient care according to company policy
  • Ensure all required teammate licensures are current
  • Provide leadership to direct/indirect reports and/or in collaboration with Regional Directors
    • Help direct/indirect reports overcome any organizational obstacles encountered during projects
    • Know, understand, implement, follow, and communicate to teammates all DaVita employment policies and procedures, awards and other opportunities within company and foster a positive work environment
    • Inspire home-facing teammates to achieve division- and local goals while promoting a strong sense of mission and values as well as personal and professional fulfillment
  • Manage all Home teammate employment activity such as hiring, promoting, job performance, evaluations and disciplinary actions with appropriate approvals as required
    • Ensure all DaVita employment processes are completed timely and in compliance with DaVita policy and any applicable laws including but not limited to ePCNs, Performance Development Reviews, teammate personnel file maintained, etc.
    • Facilitate teammate development (PDRs, coaching, mentoring, DaVita training, outside training); collaborate with direct/indirect reports to create professional development goals
  • Maintain familiarity with and communicate to teammates DaVita programs impacting teammates including but not limited to Triple Crown
  • Determine staffing plans that promote the most effective use of all teammates; ensure coverage during teammate absences
  • Understand basic wage and hour laws, federal, state and local laws and regulations, Medicare, and legislation affecting employment
  • Maintain and communicate facility emergency and disaster management programs and ability to act swiftly, confidently in an emergency


Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required.
  • Bachelor’s degree in healthcare administration, related area, or minimum of A.D.N degree from accredited school of nursing required; three-year diploma from accredited diploma program may be substituted for nursing degree
  • Current RN license in state of practice preferred
  • Minimum of two (2) years’ clinical nephrology experience required (direct patient care, program management and/or facility management) 
      • Peritoneal Dialysis and/or Home Hemodialysis experience highly preferred
      • Experience with multi-site management preferred
  • Current driver’s license, reliable transportation and vehicle insurance in state of residence required
  • Current CPR certification required and maintained without lapse
  • CNN or CDN preferred
  • Intermediate computer skills in MS Word, Excel, Outlook and PowerPoint required; ability to use the Internet effectively required
  • Other qualifications and combinations of skills may be considered at discretion of DVP and/or Group Director, Home Therapies
