Position Description

Paladina Health’s Population Health Coordinator plays an integral part in driving our clinical outcomes.  This role functions to support clinical teammates by using risk stratification to conduct proactive outreach to patients, managing referrals, as well as updating clinical records.  This individual will work in a fast-paced environment, collaborating with care teams, regional coordinators, and analytics. They will be joining an innovative team that is passionate about delivering “best in class” service with an emphasis on accuracy, efficiency and patients.

Essential Duties & Responsibilities

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive.

 

  • Convey our care model with passion to patients and external customers.
  • Utilize population health management software to risk-stratify segments of the population for various initiatives.
  • Conduct proactive outreach to patients to coordinate clinic visits, specialty visits and preventive care.
  • Utilize health coaching skills and tools to assess patient activation and encourage appropriate behavior modification.
  • Examine claims and predictive risk data to identify care opportunities, conduct outreach to obtain medical records and input in the EHR system for physician review.
  • Communicate with internal and external customers to coordinate outreach and referral efforts.
  • Manage and refine the high performing network identification process to manage outbound referrals, including training regional teammates.
  • Coordinate with clinical teams to centralize all outbound referrals, including but not limited to:
      • Pre-authorization
      • Scheduling
      • Appointment follow up
      • Obtaining records
      • Updating EMR
    • Manage referral report communications.
    • Collaborate, maintain and educate teammates on referral management process.
    • Align with the IT team to assure that systems are working as intended to help clinical teams to work more efficiently and improve patient’s health.
    • Address internal customer concerns with "best in class" service and an emphasis on accuracy and efficiency.
    Other duties as assigned/necessary.
DESIRED ATTRIBUTES
  • Flexibility to travel about 10% of the time
  • Experience with outbound and inbound patient communications and referral management

Qualifications

  • Medical Assistant, Licensed Practical Nurse, or bachelors in a health-related field preferred
  • At least 3 years’ experience working with patients and healthcare providers
  • Experience with EHR systems
  • High level of competency in Microsoft Office
