Health Coordinator plays an integral part in driving our clinical outcomes. This role functions to support clinical teammates by using risk stratification to conduct
proactive outreach to patients, managing referrals, as well as updating
clinical records. This individual will
work in a fast-paced environment, collaborating with care teams, regional coordinators,

Essential Duties & Responsibilities

proactive outreach to patients to coordinate clinic visits, specialty
visits and preventive care.

claims and predictive risk data to identify care opportunities, conduct
outreach to obtain medical records and input in the EHR system for
