Position Description

The National Head of Advance Care Planning (ACP) will serve as the point person for ACP and end of life (EOL) support programs for more than 20,000 VillageHealth patients across 46 states.  VillageHealth provides integrated care for late stage chronic kidney disease and ESRD patients.


This role will be responsible for developing and delivering training, implementing standards of care, and providing consultative services across VillageHealth (VH).  This includes ensuring an effective ACP program across VillageHealth, providing leadership for continuous program development in order to meet clinical and strategic objectives, while collaborating between departments and business units.  Additionally this resource will work with VH legal and compliance to ensure all resources and patient interactions are aligned with regulatory and legal requirements.


Primary Responsibilities

  • Provides leadership and expertise in implementing and maintaining system-wide, comprehensive end-of-life and ACP programs

  • Lead a multi-year improvement roadmap for internal capabilities, external partnerships, and objectives for improved patient care and quality of life

  • Create and provide training and ongoing support to VillageHealth nursing team to ensure all teammates are able to facilitate a EOL/ACP conversation

  • Facilitate ACP conversation with patients and their family members including the following:  identifying patient goals, ensure understanding of prognosis, identification of a Medical Power of Attorney(MPOA) or Healthcare Proxy, discussion with family and MPOA on patient preferences at EOL, education on MPOA, living wills, POLST forms, and the details including within each, and support patient in document completion and distribution

  • Provide consultation and support for interdisciplinary teammates on cases involving complex problems related to ACP, including home based care, palliative and hospice options

  • Define a process to determine which patients are entering EOL and would benefit from completing or updating their ACP and exploring EOL care options

  • Directs and coordinates the activities of the ACP program to ensure compliance with regulatory requirements and provide consultative support to clinical quality team for internal and external regulatory and quality audits

  • Provides ongoing mentoring and coaching of nursing team on ACP conversations

  • Supports the design, coordination, and delivery of educational programs for ACP and acts as liaison with internal and external stakeholders

  • Assist with developing and revising policies and protocols related to ACP

  • Maintain knowledge of current trends and practice in ACP community



Required Qualifications
  • Licensed Social Worker required
  • 7-10 years of experience in patient facing healthcare setting
  • Experience facilitating advance care planning discussions, care coordination, bereavement and/or palliative care
  • Understanding of state notary and witness requirements as they relate to Living Wills and MPOA
  • Strong organizational skills and ability to coordinate to meet program objectives
  • Self-motivated, excellent time management skills, experience using clinical and ethical judgment
  • Experience with research, quality improvement, and management
  • Excellent communication skills with management, physicians, patients and staff
  • Knowledge of Federal and State laws and Joint Commission / NCQA regulations relating to clinic operations
  • Experience working with kidney care patients or other chronically ill patient populations, grief and loss, or in a hospice setting
Preferred Qualifications
  • Masters of Social Work or Certified Hospice and Palliative Nurse (CHPN)
  • Prior palliative care or hospice experience
  • Experience working with Medicare beneficiaries
