DaVita Integrated Kidney Care (DaVita IKC) provides comprehensive chronic kidney disease care and complex chronic condition management for DaVita dialysis patients. The goal is to help patients focus on their overall health so they can spend more time living a healthier life, and less time trying to coordinate their health care needs. The IKC team is an extension of in-center care teams, working together in active collaboration to provide holistic patient care. This includes supporting patients through the kidney disease process, as well as comorbid condition management such as diabetes, cardiac disease, hypertension, and mental health and well-being assistance. DaVita IKC operations consist of clinical and non-clinical teammates fully dedicated to helping patients focus on their overall health and improve patient quality of life.

DaVita IKC Clinical Careers
Our team of nurse practitioners, nurses, coordinators, physicians, and more, works across the health care spectrum, coordinating care before and during the transition to dialysis or transplant, as well as inside and outside of the dialysis clinic to achieve better clinical outcomes and improve patient quality of life.

Nurse Practitioner
Work closely with patients to provide comprehensive assessments including ordering and interpreting diagnostic tests and prescribing medications and other treatments.
Registered Nurse, ​​​​​​​Case Manager
Provide the coordination of care between the patient, family, physician, dialysis provider and care team,
the community, and Regional Operations Manager. 
Care Coordinator
The single point of contact to coordinate
resources such as program enrollment and referrals and appointments for DaVita IKC patients. 
Integrated Care Assistant
Provide administrative support for the DaVita IKC team by collecting and managing all medical record information.  

DaVita IKC Non-Clinical Careers
Regional Operations Manager
Provide leadership and direction to the DaVita IKC program in a designated market or geographic region. This includes financial and risk management, quality initiatives, outcomes, and performance measurements.
Regional Operations Supervisor
Supervise the daily operations
of a team of care coordinators in a designated market or geographical region.
