Providing Holistic & Integrated Patient Care

As a nurse practitioner (NP) at DaVita, you’ll provide holistic, integrated care to improve the quality of life for patients with chronic kidney disease (CKD), whether they are in the early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant or receiving life-sustaining dialysis. You'll work as part of the DaVita Integrated Kidney Care (DaVita IKC) team in collaboration with doctors, registered nurses, and other members of a care team, providing coordinated and individualized care to help delay chronic kidney disease progression, smooth the transition from CKD to end stage kidney disease (ESKD) and optimize ESKD treatment for our patients.

DaVita Integrated Kidney Care (IKC)
DaVita IKC provides comprehensive chronic kidney disease care and complex chronic condition management for DaVita dialysis patients. The goal is to help patients focus on their overall health so they can spend more time living a healthier life, and less time trying to coordinate their health care needs. The IKC team is an extension of in-center care teams, working together in active collaboration to provide holistic patient care. This includes supporting patients through the kidney disease process, as well as comorbid condition management such as diabetes, cardiac disease, hypertension, and mental health and well-being assistance.
