A social worker (MSW) at DaVita works mainly in a clinical setting, but can sometimes accompany an RN to a home visit or skilled nursing facility. Life on dialysis can be a difficult transition for many patients, and you'll be there to support and advocate for them as you help them cope with the effects of chronic kidney disease. Working as part of a care team, you'll provide detailed case management, including patient plan assessment and evaluation, as well as best practices for monitoring the treatment process and health care needs of patients. This includes educating patients and helping them make informed decisions regarding their plan of care. You'll help patients address the psychological, emotional, and social challenges that affect their quality of life, and equip them with information and resources for managing the financial demands of chronic kidney disease.
