Support Beyond Treatments

Life on dialysis can be a difficult transition for many patients, and as a social worker (MSW) at DaVita, you will be there to support and advocate for them as they navigate the challenges of life on dialysis due to chronic kidney disease.

You will primarily work in a clinical setting but may also accompany a Registered Nurse (RN) on home visits or to skilled nursing facilities. Collaborating with a care team, you will conduct thorough case management, assess patient plans, monitor treatment progress, and provide education on healthcare options. Your focus will be on addressing the psychological, emotional, and social challenges that patients may face that can affect their quality of life and assist them with information and resources for managing the financial demands of chronic kidney disease.

Home Dialysis

The Social Worker (MSW) conducts psychosocial assessments to support dialysis patients, addressing strengths, goals, and needs. They work with the Interdisciplinary Team to provide clinical interventions, improve outcomes, and educate patients on rights, responsibilities, and resources. MSWs also screen for depression, assist with modality choice and adjustments, and guide advance care planning.

Home Dialysis 

Some dialysis patients don’t dialyze in a center but perform peritoneal dialysis (PD) or hemodialysis (HHD) at home. PD and HHD social workers’ responsibilities are the same as in-center hemodialysis social workers, with the addition of helping our home patients adjust to performing dialysis independently at home, including coordination with care partners who may be assisting our patients.

In addition to in-center hemodialysis and home dialysis responsibilities, some of our social workers collaborate and coordinate care with a Skilled Nursing Facility (SNF) team to support patients dialyzing during either a short or long-term SNF stay.
